In order to save time when you arrive, and to guarentee use of the Bubble Footballs, you may want to complete your Risk Acknowledgement in advance. You must be over 18 years old to complete the form. If you are under 18, your parent or guardian must complete the acknowledgement form on your behalf. Please note that the minimum age for Bubble Football at AJD is 7 years old. In consideration, and as a condition of being allowed to participate in the services and activities including when inside AJD Bubbles, provided by AJD Football Coaching Ltd; I hereby acknowledge and agree to the following: 1. I declare that by signing this agreement on behalf of a child under the age of 18 years, I am the Child’s Parent or Legal Guardian, or I have obtained explicit consent from the Childs Parent or Legal Guardian to complete this Waiver on their behalf. 2. We (parent/guardian and child/or child) agree to attend the Safety Briefing prior to undertaking any activity. This must be done on the day of using AJD Bubble Football. 3. I agree that I will carry out the activity in accordance with the instructions given in the Safety Briefing and Activity Rules at all times. 4. I confirm that I am good physical health and have no medical conditions or injuries that may affect my ability to partake in the Activities. 5. I accept that the Activities that I intend to partake are potentially dangerous and carry a risk of personal injury.. 6. I agree to carry out Activities within my skill level and comply with all instructions given by AJD Football Coaching staff. 7. Prior to commencing in the Activities, I will remove all jewellery, mobile phones and other valuables. I acknowledge that I have read and fully understand the above.
